ABOUT THE SERVICE

The service connection intends to help concessionaires avail of safe and potable water supply at least possible cost. Water service installation is made after site inspection, payment of all applicable fees and submission of all requirements at the CWD Office.

WHO CAN AVAIL OF THE SERVICE

Any person of legal age, residing within the service area of Concepcion Water District

SCHEDULE OF AVAILABILITY OF SERVICE:

Monday-Friday

8:00 a.m-5:00 p.m.

NO NOON BREAK

REQUIREMENTS TO AVAIL THE SERVICE

  1. Valid government issued ID’s
  2. 1pc. 2”x 2” ID picture
  3. Proof of ownership

For Private Property (any of the following)

✓ Photocopy of Barangay Certificate as bonafide resident of the area

✓ Photocopy of Land Title

✓ Photocopy of Deed of absolute sale

For Renter / Lessee of a Privately-Owned Lot (any of the following)

➢ Photocopy of current and notarized Lease Contract

➢ Photocopy of Land Title

➢ Notarized Authorization / Consent from the Lot Owner / Lessor or Special Power of Attorney

➢ Notarized Affidavit of Undertaking

  1. Location/Sketch Plan
  2. Attend Orientation on New Service Connection with Certificate of Attendance.
  3. Fully accomplished Water Service Contract and Service Application/Construction Order.

DURATION OF TRANSACTIONS

* Customer In – Office Transaction – 1 hour and 10minutes * Under normal condition:

Duration: 3-5 days working days

Inspection: 1 to 2 days from date of filing of application

Tapping : 3 to 5 days under normal condition Notes:

1) This work standard excludes those applications requiring permits from DPWH, which processing may take longer.

2) Validity of Service Application and Connection Order (SACO) form (Form 1) is 1 year. 3) Validity of orientation attendance certificate is 1 year.

HOW TO AVAIL OF THE SERVICE
